We've had it in our household for about a year from the local cable company company called Cablevision.
Unlimited calling nationwide for $39.95 ($34.95 for 1st 6 mos).
The wife gets to call her mom on the left coast and I don't get a heart attack paying the phone bill. Works just like a regular phone with caller id, answering system, call forwarding, call waiting etc. No sales calls (they haven't published their subscriber list (yet).

Downside: No enhanced 911 (not yet), phone is dead when power is out (we also have cell phones), must also subsribe to their web service as well ($49.95), and we had to change to a new phone number.

Rather than pay more for our local phone service, we have the minimum we can get ($9.95/mo) and use the cell phones for long distance. Since we had the cell phones anyway, and unlimited long distance is included, why not?

My wife and my mother have hour long phone conversations with our kid who's going to school in Hawaii. Of course, when he talks to me it's just - send money!
